Job summary

Oak Street Health in New York, NY seeks a Nurse Practitioner (Spanish Preferred) to deliver value-based primary care for adults on Medicare. You will work on a collaborative care team, see patients independently, and coordinate with physicians to keep patients healthy and out of the hospital.

The role includes annual wellness visits, health risk assessments, care coordination, and educating patients about conditions and treatments, while participating in Oak Street Health promotional activities

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP
• Active, non-probationary NP license
• Active DEA license
• US Work Authorization
• Bilingual proficiency in Spanish

Responsibilities

• Provide exceptional primary care to adults on Medicare.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with other providers, facilities, and agencies.
• Population health leadership with the care team to ensure screening and preventive care.
