Job summary

Oak Street Health in New York is seeking a Nurse Practitioner (Creole preferred) to deliver patient-centered, value-based primary care to adults on Medicare. You will work on a collaborative care team, conduct wellness visits, coordinate with specialists, and educate patients about their conditions and care plans.

The role reports to the Center Medical Director and offers a comprehensive benefits package with competitive pay. Full-time, on-site in New York.

Qualifications

• Master of Science in Nursing or doctoral NP program completion.
• National certification in Family NP, AG-PCNP, ANP, or Gerontological NP.
• Active, non-probationary NP license in the state.
• Active DEA license and US work authorization.

Responsibilities

• Provide effective, equitable value-based primary care to adults on Medicare.
• See patients independently and collaborate with physicians as needed.
• Conduct office visits for routine and acute issues and annual wellness visits.
• Coordinate care with other providers and care managers.
• Engage in population health activities and health risk assessments.
