Job summary

Chinatown Community Development Center seeks an Intensive Case Manager to provide intensive case management services in Spanish to formerly homeless residents in supportive housing in San Francisco. You will address complex medical, mental health, and substance use histories and collaborate with housing staff and community partners.

The role requires a social work background, bilingual English/Spanish, and a passion for affordable housing and social services.

Qualifications

• Bachelor’s Degree in Social Work or related field; three years of intensive case management experience.
• Bilingual English/Spanish; strong communication with diverse populations.
• Crisis de-escalation and harm-reduction experience; psychoeducational groups beneficial.

Responsibilities

• Conduct intakes and assessments to establish strengths, needs and goals for residents.
• Provide case management and counseling to support action plans and progress.
• Assist residents with benefits, income, and community resources.
• Provide counseling on mental health, substance use, and home maintenance; offer groups if requested.
• Coordinate with Property Management to orient residents to lease rules and housing stability.
