Qualifications

• Associate degree in Behavioral Health, Health Sciences, or related field.
• Active Agency Affiliated license within 60 days of hire.
• Must pass a TB test and have a valid State Driver's license.

Responsibilities

• Provide case management to mentally ill, homeless, and substance-using Sea Mar clients.
• Coordinate referrals and services, and maintain client files as required by state and clinic.
• Conduct intake screenings and develop Client Service Plans for activities.
• Engage in regular interdisciplinary team case reviews and follow agency policies.
