Job summary

The Intensive Case Manager at Chinatown Community Development Center provides intensive and culturally competent case management in Spanish to formerly homeless residents at the Broadway Sansome apartments and Philip C Chin Commons, supporting housing stability and access to health and social services.

This full-time, nonexempt role requires sensitivity to diverse communities and collaboration with housing, health, and community partners in a nonprofit setting.

Qualifications

• Bachelor’s Degree in Social Work or related field.
• Three years of experience providing intensive case management services.
• Bilingual and biliterate in English/Spanish.
• Knowledge of, and sensitivity to, the needs of low-income populations.
• Experience with crisis de-escalation and the harm-reduction model.
• Experience with support or psychoeducational groups.

Responsibilities

• Conduct intakes and assessments to establish strengths, needs, and goals to formulate individualized action plans for residents.
• Provide case management and counseling services to establish goals, support individualized action plans, and track resident progress towards meeting goals.
• Provide assistance and referrals to help residents obtain/maintain benefits, increase income, and connect with community resources.
• Facilitate individualized counseling on mental health, substance use, and home maintenance, and groups if requested.
• Work with Property Management to orient residents to lease rules and provide ongoing support.
