Job summary

Sea Mar Community Health Centers in Washington seeks a Behavioral Health Specialist to provide primary community support and case management to mentally ill, homeless, and substance abuse clients. You will conduct intake screenings, coordinate service plans, and refer clients to community resources while maintaining files and collaborating with an interdisciplinary team.

Qualifications

• Associate degree in Behavioral Health, Health Sciences, or related field.
• Two years of related work experience required.
• Eligible for Agency Affiliated Counselor Registration from the State Department of Health.
• Active Agency Affiliated license within 90 days of hire.
• Must have and maintain a TB test; valid state driver's license.

Responsibilities

• Provide care appropriate to the age of patients served.
• Conduct intake screenings and develop Client Service Plans.
• Make referrals and assist client self-referral.
• Maintain regular contact with clients and provide advocacy.
• Maintain client files documenting case management activities.
• Collaborate with interdisciplinary team for reviews.
• Follow agency policies and applicable regulations.
• Complete daily record keeping as directed.
• Report unusual incidents to the Program Manager.
• Fulfill mandated reporter duties for child/elder abuse.
• Assist clients in accessing services and transportation.
• Observe and assess clients’ needs and refer as needed.
