Job summary

Maxim Healthcare is seeking a compassionate Intake Coordinator to join our team, ensuring timely patient care and accurate intake processing. You will work with the Client Coordinator to manage referrals, document service needs, and inform patients about available services.

Ideal candidates have strong English/Spanish communication, organizational skills, and proficiency with MS Office. Travel up to 25% is required, with weekly pay and comprehensive benefits.

Qualifications

• High School Diploma or equivalent required.
• LPN licensure and/or college degree preferred.
• Minimum of 1 year of relevant experience preferred.
• Ability to travel up to 25%.
• Strong organizational and time management skills.
• Excellent communication and customer service abilities.
• Professional telephone manner and interpersonal skills.
• Proficiency in Microsoft Office Suite (Word, Excel, Teams).
• Ability to lift up to 15 pounds.
• Strong written and verbal communication in English and Spanish required.

Responsibilities

• Capture and communicate patient referral information to management teams.
• Process and document incoming patient needs, service requests, and changes.
• Inform patients accurately about available services.
• Collaborate with operations to schedule appropriate personnel promptly.
• Assist with new case openings in patient homes.
• Record and report patient and employee concerns to appropriate parties.
• Enter patient data into the system of record per company policies.
• Support office staff with routine administrative duties.
• Perform other duties as assigned.
