Job summary

Unicare Home Care seeks an Intake Coordinator to manage referrals and admit patients into active home care cases.

You will handle inquiries by phone and email, gather medical, demographic and insurance information, and ensure forms are completed accurately, assisting with supporting documentation as needed.

Strong bilingual English/Spanish communication is required, and you will coordinate with families, referral sources, and vendors while maintaining current admissions records.

Qualifications

• Excellent written and verbal communication skills.
• Knowledge of the different facets of home care.
• At least 1 year of experience as an Intake Coordinator in a home care agency.
• Bilingual English/Spanish is required.

Responsibilities

• Convert referrals into active home care cases.
• Address inquiries via phone and e-mail and collect medical, demographic and insurance information.
• Ensure referral forms are completed correctly and assist with supporting documentation.
• Advise appropriate staff of new admissions and enter cases into scheduling system.
• Maintain accurate records of admissions, discharges and changes to caseload.
• Act as liaison with family members and referral sources.
• Serve as point of contact with contracted vendors.
