Personal Touch Home Care of N.Y. Inc. is seeking an Intake Specialist for Home Care in Brooklyn, NY. The role handles initial intake for patients requesting home care services and requires strong coordination with clinical and admissions teams.

Candidates must have at least 1 year of home care experience and strong communication skills. Bilingual English-Spanish or English-Creole is preferred. This is an in-person role with comprehensive benefits and a focus on quality patient care.

Qualifications

• Must be 18 years or older.
• High School Diploma or GED required.
• Experience with HHAeXchange a plus.
• Prior home health care experience a plus.
• Basic understanding of Medicaid, Medicare and long-term care insurance.
• Knowledge of EPACES, BOLT, HHAeXchange preferred.

Responsibilities

• Receives and processes referrals via multiple channels (marketers, call-ins, web leads, walk-ins).
• Answers phones and gathers required information for admissions for referrals under MLTCs, MCOs, private pay, and Medicaid approvals.
• Inputs data and maintains notes in the referral management system.
• Maintains accurate records of intake activities and documentation.
• Verifies insurances and long-term benefits.
• Performs basic evaluation of clients' needs to determine home care eligibility.
• Communicates with patients and families throughout the referral lifecycle.
• Coordinates with contract agencies for service status and authorizations.
• Alerts clinical and coordination teams of upcoming enrollments for staffing.
• Monitors emails and referral workflow daily.
