Job summary

Alignment Health seeks a compassionate bilingual Health Coach & Educator to provide in-home support, wellness coaching, and health education to members in Maricopa County, AZ. This field-based role operates Monday–Friday, 8:00 AM–5:00 PM PST with local travel and door-to-door mileage reimbursement.

Requirements include bilingual Spanish proficiency, 1+ year as a wellness health coach or educator, and a CA driver’s license. Associate’s degree or two years of relevant experience are acceptable.

Qualifications

• Bilingual Spanish required: written, read, and spoken.
• 1+ year of experience as a wellness health coach or educator.
• California Driver’s License (valid, unrestricted).
• Proficient with Microsoft Office (Excel, Word, PowerPoint).
• Knowledge of health, wellness, nutrition, exercise, and behavior change principles.

Responsibilities

• Conduct 6–8 in-home visits daily within the service area.
• Provide wellness coaching and health education to support behavior change.
• Partner with clinicians and social workers to develop member action plans.
• Connect members with community resources and social services.
• Support member engagement, retention, and satisfaction; coordinate care with teams.
