Job summary

Alignment Health seeks a compassionate Bilingual Health Coach & Educator to provide in-home wellness coaching and health education to members across California. You will build trusted relationships with members, caregivers, and families while helping individuals adopt healthier habits and access community resources.

This field-based role requires daily travel, independent work, and strong communication skills.

Qualifications

• Bilingual Spanish required (read, write, speak).
• Minimum 1+ year as a wellness health coach or educator.
• California Driver's License and reliable transportation.
• Ability to work independently and remotely within territory.
• Strong communication and organization skills; HIPAA knowledge.

Responsibilities

• Conduct 6-8 in-home visits daily across the service area.
• Provide wellness coaching and health education to support behavior change.
• Partner with clinicians and social workers to develop member action plans.
• Connect members with community resources and social services.
• Support member engagement, retention, and satisfaction.
• Coordinate care with internal clinical teams and external partners.
• Deliver health education materials to members, caregivers, and families.
