Job summary
Alignment Health is seeking a compassionate bilingual Health Coach & Educator to provide in-home support, wellness coaching, and health education to members across Maricopa County, Arizona. This field-based role builds trusted relationships with members, caregivers, and families while helping individuals adopt healthier habits and access community resources.
Daily responsibilities include 6-8 in-home visits, collaboration with clinicians to develop member action plans, and delivering health
Qualifications
- 1+ year experience as wellness health coach or educator
- Valid driver’s license and reliable transportation
- Ability to travel within Maricopa County
- Ability to work independently and remotely
- Knowledge of health, wellness, nutrition, exercise and behavior change
- HIPAA compliance and confidentiality
Responsibilities
- Conduct 6-8 in-home visits daily across service area
- Provide wellness coaching and health education
- Partner with clinicians to develop member action plans
- Connect members with community resources
- Support member engagement and satisfaction
- Coordinate care with clinical teams and partners
- Deliver health education materials to members and families
