Job summary

Alignment Health is seeking a compassionate, motivated Bilingual Health Coach & Educator to provide in-home support, wellness coaching, and health education to members throughout Maricopa County, Arizona. Primarily field-based with local travel, this role requires Spanish proficiency and independent work to support healthier lifestyles.

Qualifications

• Bilingual Spanish, written and spoken.
• At least 1 year experience as wellness health coach or educator.
• Current, valid California driver’s license; travel within territory.

Responsibilities

• Conduct approximately 6-8 in-home visits daily in assigned area.
• Provide wellness coaching and health education to support behavior change.
• Partner with clinicians and social workers to develop member action plans.
• Connect members with community resources and social services.
• Support member engagement, retention, and satisfaction.
• Coordinate care with internal teams and external partners.
• Deliver health education materials to members, caregivers, and families.
