Job summary

Alignment Health is seeking a compassionate, motivated Bilingual Health Coach & Educator to provide in-home support, wellness coaching, and health education to members. This field-based role requires daily in-home visits and travel throughout the assigned territory, primarily in California.

The position emphasizes independent work, building relationships with members and caregivers, and partnering with clinicians to improve health outcomes.

Qualifications

• Bilingual Spanish, must be able to write, read, speak.
• Minimum 1+ year of experience as a wellness health coach or educator.
• Current, valid California Driver's License.
• Reliable personal transportation and ability to travel throughout the assigned territory.
• Ability to maintain HIPAA compliance and confidentiality.

Responsibilities

• Conduct approximately 6-8 in-home visits daily in the assigned service area.
• Provide wellness coaching and health education to support behavior change and healthier lifestyles.
• Partner with Advanced Practice Clinicians and Social Workers to develop member action plans.
• Connect members with community resources and social services.
• Support member engagement, retention, and satisfaction; coordinate care with internal clinical teams and external partners.
• Deliver health education materials to members, caregivers, and families.
