Job summary

Alignment Health is seeking a compassionate Bilingual Health Coach & Educator to deliver in-home wellness coaching, health education, and community resource access for members in the Fresno, CA area. This field-based role emphasizes independent work, travel, and building trusted relationships with members, families, and caregivers.

You will partner with clinicians to create member action plans and support healthier habits.

Qualifications

• Bilingual Spanish: write/read/speak.
• Minimum 1+ year as wellness health coach or educator.
• California Driver's License required.
• Reliable personal transportation and ability to travel Fresno area.
• HIPAA compliance knowledge.

Responsibilities

• Conduct 6-8 in-home visits daily in assigned area.
• Provide wellness coaching and health education to support behavior change.
• Collaborate with Advanced Practice Clinicians and Social Workers to develop member action plans.
• Connect members with community resources and social services.
• Support member engagement, retention, and satisfaction.
• Coordinate care with internal clinical teams and external partners.
• Deliver health education materials to members, caregivers, and families.
