Job summary

Alignment Health is seeking a compassionate, motivated bilingual Health Coach & Educator to provide in-home support, wellness coaching, and health education to members in Maricopa County, Arizona. This field-based role involves building trusted relationships with members, caregivers, and families while helping individuals adopt healthier habits and improve overall well-being.

Responsibilities include daily local travel, delivering health education materials, connecting members with community

Qualifications

> In-home wellness coaching and health education to members.
> Travel to assigned territory for daily visits.
> Bilingual Spanish required (written, reading, speaking).
> Minimum 1+ year of wellness health coach or educator experience.
> Reliable transportation and ability to travel locally.
> Ability to work independently and remotely when needed.
> Proficiency with Microsoft Office (Excel, Word, PowerPoint).
> Knowledge of health, wellness, nutrition, exercise, and behavior change principles.

Responsibilities

Provide in-home wellness coaching and health education.
Connect members with community resources and social services.
Deliver health education materials to members, caregivers, and families.
