Job summary

The Commonwealth of Massachusetts, Department of Developmental Services, seeks a Human Services Coordinator I (Bi-Lingual English/Spanish) for the Springfield/Westfield Area Office. You will coordinate and facilitate Individual Service Plans (ISPs) for clients on your caseload and monitor services across DDS-funded and community providers.

You will advocate for individuals and families, document care, maintain records, and coordinate with providers, guardians and agencies.

Qualifications

• At least three (3) years of full-time or equivalent experience in human services or social work.

Responsibilities

• Monitor safety and well-being of assigned individuals.
• Provide advocacy in human, civil and legal rights; facilitate team meetings.
• Maintain regular contact with individuals, providers and families.
• Arrange crisis intervention and follow-up planning as needed.
• Perform related duties including Participant Directed Services.
