Job summary

Qualifications

• Selective Placement Factor training as part of the application process.

Responsibilities

• Serve as case manager and service coordinator for assigned clients.
• Facilitate collaboration among agencies, providers, and clients to coordinate care.
• Develop service plans and monitor progress to mitigate barriers and promote self-sufficiency.
• Assist Spanish-speaking clients by translating documents and communicating in English/Spanish.
