Job summary

Episcopal Community Services (ECS) seeks an Interim Housing Case Manager to carry a caseload of up to 25 participants, conducting assessments and developing Individual Service Plans, meeting clients at least weekly to implement those plans, and coordinating with Program Leadership to ensure compliance.

This fully onsite role focuses on housing, income, health services, and discharge planning, requiring strong communication, trauma-informed practice, and ability to work with diverse populations

Qualifications

• Associate degree in psychology, counseling, social work, or a related field.
• CCSF Community Health Worker Certificate and 2–4 years of relevant work experience.
• High school diploma/GED and 4 years of relevant work experience.
• Experience with person-centered harm-reduction and trauma-informed approaches.
• Strong background working with individuals experiencing homelessness.
• Excellent communication and organizational skills.
• Proficiency with Microsoft Office and electronic record systems.

Responsibilities

• Manage a caseload of up to 25 participants.
• Perform Individual Service Need assessments.
• Provide back-up coverage for other case managers, as directed by the Site Manager.
• Collaborate with the participant to develop an Individual Service Plan focused on housing needs.
• Create an action plan to help the participant achieve goals.
• Report participant progress to Program Leadership as required.
• Provide case management linkage services for housing, income, primary care, substance use treatment, behavioral health, legal, employment, clothing, and other resources.
• Meet with participants at least weekly and document sessions.
• Maintain accurate records by entering documentation into electronic systems.
• Assist participants with discharge planning and referrals as needed.
• Adhere to agency, state, and funder regulations.
• All other duties as assigned.
