Job summary

Episcopal Community Services (ECS) seeks a Part Time Case Manager to perform intake assessments, engage residents onsite, and develop client-centered Service Plans. The role emphasizes in-person interaction, crisis response, and coordination with housing and support programs.

Ideal candidates have a mix of education and experience in social services, strong documentation skills, and the ability to work as part of a multidisciplinary team in a high-demand environment.

Qualifications

• High School Diploma plus experience requirements or related degrees.
• Senior role prefers MA/MS or BA/BS with relevant experience.
• CHW/CADC certificates acceptable for entry.

Responsibilities

• Perform intake assessment for residents at move-in.
• Outreach to residents and engage in crisis responses.
• Develop individualized Service Plans with residents.
• Provide referrals to in-house or community resources.
• Assist with eligibility for benefits and enrollment.
• Maintain confidential, complete documentation.
