Job summary

Episcopal Community Services (ECS) is seeking a Case Manager to carry a caseload of about 35 participants and deliver in‑person services at ECS program locations. You will assess needs, develop Individual Service Plans, and work closely with leadership to ensure compliance with regulations.

The role emphasizes housing support, coordinated services, and strong documentation practices. A background in harm‑reduction and trauma‑informed approaches is preferred, with a commitment to serving unhoused

Qualifications

• High school diploma or higher in a related field with relevant experience.
• Experience in housing, trauma‑informed care, harm‑reduction.
• Experience documenting services in electronic record systems.

Responsibilities

• Manage a caseload of approximately 35 participants.
• Perform Individual Service Needs Assessments.
• Collaborate with participants to create and implement Individual Service Plans focusing on housing.
• Create action plans to help participants achieve goals.
• Report participant progress to Program Leadership as required.
• Provide linkage services for housing, income, primary care, and behavioral health.
