Episcopal Community Services (ECS) in California seeks a dedicated case manager to provide in-person support to residents. You will perform intake assessments, develop individualized service plans, facilitate groups, and coordinate referrals to housing, health, and social services.

This role requires strong documentation, de-escalation skills, and collaboration with a multidisciplinary team. Bilingual Spanish is desirable.

Qualifications

• MA/MS with 2+ years of relevant experience.
• BA/BS with 4+ years of relevant experience.
• CHW Certificate with 4+ years of experience.
• Knowledge of local social services for underserved populations.
• Patience, compassion, and empathy for participants.

Responsabilités

• Perform intake assessment of residents on move-in day, reviewing health, psychosocial history, finances, housing, and more.
• Outreach to residents on caseload regularly and during crises.
• Develop and implement individualized Service Plans tailored to goals and needs.
• Provide referrals to in-house or external resources for substance use, health, housing, and other services.
• Assist with budgeting and money management planning for residents.
• Advocate with Property Management to prevent eviction when possible.
• Assist with conflict resolution to maintain a respectful living environment.
• Conduct twice-weekly Wellbeing Checks and emergency safety checks as needed.
• Develop and facilitate at least one weekly therapeutic group at the site.
• Maintain confidential, complete documentation and complete required paperwork timely.
• Assist in discharge planning and referrals for continued case management.
