Job summary

Caring for People Services in Grand Island, Nebraska, is seeking an Assistant Care Manager to support the Care Manager in coordinating high-quality, non-medical home care services across an assigned client caseload. This in-office role requires travel to clients' homes, on-call rotation, and occasional after-hours

Qualifications

• High school diploma or equivalent; associates degree preferred.
• 2+ years in home care, healthcare, human services, or a related field.
• Experience in supervision, caregiver coordination, or team leadership is a plus.
• Ability to pass a thorough background check.
• Ability to provide emergency or temporary direct-care coverage.

Responsibilities

• Assist the Care Manager with coordinating services for an assigned client caseload.
• Assist in client intake, service initiation, ongoing service coordination, reassessments, and discharge processes.
• Assist with gathering information for client assessments and individualized care plans.
• Assist with client visits, quality-assurance contacts, and service observations as assigned.
• Communicate professionally with clients, families, providers, and payers.
• Maintain accurate, timely documentation of client activities and communications.
