Job summary

The role requires travel (vehicle needed) and a High School diploma, with full-time employment in a care-management setting. You will conduct visits, documentation, and participate in ongoing training as part of a dedicated team.

Qualifications

• Background in Care Management or MLTC services.
• Ability to travel and perform home visits; vehicle required.
• Excellent interpersonal and communication skills with diverse populations.
• Bilingual Cantonese/Mandarin preferred.

Responsibilities

• Conduct face-to-face visits and maintain care coordination.
• Schedule visits and complete required documentation.
• Participate in team meetings and ongoing training.
• Meet weekly productivity expectations.
