Job summary

The City of Lincoln in Brooklyn, NY is seeking a Care Manager Assistant to provide face-to-face visits, ongoing support and care coordination for members. You will conduct visits, review service plans with members and families, and coordinate covered and non-covered benefits.

Fieldwork is required. Candidates should be able to travel extensively (home visits), possess strong interpersonal and communication skills, and have a high school diploma.

Qualifications

• Able to travel and conduct home visits
• Knowledge of member assessment procedure
• Excellent interpersonal skills – ability to speak effectively with members and employees of the agency
• Demonstrates the ability to effectively communicate information with members of all ages and socioeconomic levels
• Ability to read and interpret documents, understand and execute workflows

Responsibilities

• Conduct multiple types of mandatory face to face visits based on need
• Schedule members due for face-to-face visits
• Meets weekly productivity expectations
• Completes necessary documentation, scripts, activities and follow ups – ensure all items are addressed at time of visit
• Understands and supports company wide goals based on direction
• Participate in weekly team meetings and ongoing training
• Other duties as identified as a necessary part of employee’s role
