Job summary

The Puerto Rican Cultural Center in Chicago, IL is seeking a Case Manager to provide direct and indirect services supporting follow-up, referrals, and data entry for HIV, behavioral health, and substance use prevention and care.

You will engage frontline staff, work with diverse communities, explain PrEP and U=U, maintain confidentiality under HIPAA, and collaborate with hospitals and community partners to link clients to care.

Qualifications

• Experience providing outreach and working with underserved populations.
• Knowledge of HIV and community resources, preferred
• 1 year of experience working in an HIV-related field or social services, preferred
• HIV Test Counselor or any other HIV prevention training, preferred

Responsibilities

• Provide support, advocacy, information, resources, and referrals to participants to ensure linkage to care.
• Train and conduct HIV testing and counseling.
• Enter data and complete required documentation accurately and promptly.
• Collaborate with a multidisciplinary team while also interacting with a diverse group of clients, visitors, and external and internal contacts.
• Explain PrEP and U=U to those living with HIV and refer to PrEP.
