Job summary

Casa Esperanza, Inc. in Boston, MA is seeking a Tu Bienestar Case Manager to link clients to care, engage them in care, and help achieve viral suppression.

The role coordinates admissions for Ryan White clients and provides comprehensive support across medical, dental, counseling, and social services in a team-based, integrated model. The position requires strong organizational skills, experience with HIV-related case management, and a valid MA driver’s license.

Qualifications

• Ability to work independently and as part of a team.
• Experience conducting outreach to HIV, addiction, mental illness, trauma, and violence among at-risk individuals.
• Ability to provide HIV education, group facilitation, service planning, and health navigation.
• Knowledge of care coordination and community resources.
• Familiarity with the substance use field and motivational counseling techniques.
• Knowledge of co-occurring disorders.
• Strong writing and organizational skills.
• Valid Massachusetts driver's license.

Responsibilities

• Coordinate admissions and intakes for Ryan White clients with internal/external programs.
• Ensure eligibility for Part A services and document in records.
• Perform initial assessments and create individualized service plans.
• Conduct ISP reassessments and track service utilization.
• Assist clients in selecting primary care providers and obtaining health insurance.
• Educate medical personnel about HIV issues and coordinate care.
• Coordinate access to medical and behavioral health services for continuity of care.
• Provide onboarding and annual HIV/AIDS education to staff.
• Participate in monthly BPHC meetings and RW program quality projects.
• Represent the agency in community events and outreach.
• Facilitate HIV education at Casa Men’s Program and Latinas y Niños Center.
