Qualifications

• Bachelor’s degree in Social Work, Human Services or related field required.
• BLS Certification required.
• One year of Case Management experience required.

Responsibilities

• Provides ongoing case management for patients living with HIV/AIDS.
• Coordinates health and social services for clients.
• Empowers clients for self-advocacy as appropriate.

Skills

Ability to work independently

Basic business writing skills

Basic skills in Office 365

Ability to prioritize and organize

Bilingual/Bicultural Haitian Creole and/or Spanish

Education

Bachelor’s degree in Social Work or related field

BLS Certification

One year of Case Management experience
