Job summary

Chicanos Por La Causa is hiring a bilingual Medical Case Manager in Phoenix, AZ to help lead the fight against HIV through education and care. You will provide direct services to clients living with HIV, coordinate with community supports, manage appointments, and act as a clinical liaison to medical services.

You will conduct needs assessments, implement treatment plans, facilitate referrals, and communicate effectively in both Spanish and English to help clients navigate barriers to care.

Qualifications

• Bilingual English/Spanish skills required to communicate with clients in both languages.
• Knowledge of HIV/AIDS and ability to educate clients about HIV/AIDS and STDs.
• Flexibility to work a schedule that meets program requirements.
