Job summary

Pair Team is seeking a Field Care Coordinator to bring care directly to patients in the community across Alameda County. In this highly active, in-person role, you will manage a busy field schedule, conduct home visits, and onboard new patients to ensure access to resources and adherence to care plans.

The role requires in-person outreach five days per week, with a typical monthly target of around 120 visits.

Qualifications

• Bilingual in English and Spanish is required.
• 1+ years of field-based (in-person) case management or care coordination.
• Physical location in or near Alameda County with local knowledge.
• Reliable transportation for regular field visits and a valid driver's license.
• Strong cultural fluency and ability to build trust with diverse populations.
• High level of empathy and collaborative working style.

Responsibilities

• Manage a high-volume, field-based caseload with ~30 in-person visits per week.
• Travel throughout Alameda County to meet patients where they are.
• Support community-based events and provide care coordination for complex needs.
• Serve as primary contact for members with medical, behavioral, and social needs.
• Meet members in homes or partner sites to onboard and deliver care packages.
• Document interactions and coordinate services across care teams.
