Job summary

Pair Team seeks a Field Care Coordinator to bring care directly to patients in Alameda County. You will conduct in-home visits, deliver groceries or care packages, and connect patients with essential resources while coordinating with the care team.

You will travel in the community five days a week, manage a busy schedule, and support orientation and onboarding for new patients. Bilingual English/Spanish is required.

Qualifications

• 1+ years of field-based (in person) case management or care coordination.
• Physical location in or near Alameda County with familiarity of the local community.
• Reliable transportation for regular field visits to patient homes and partner sites.
• Valid driver’s license and current auto liability insurance.
• Bilingual – English/Spanish.
• Strong organizational and time-management skills.
• Experience with Google Suite, Slack, CRM databases, and basic Excel/Word/Email/Video conferencing.

Responsibilities

• Manage a high-volume, field-based caseload with ~30 in-person visits per week.
• Travel throughout Alameda County meeting patients where they are.
• Support community events and care coordination for complex cases.
• Serve as primary contact for members with complex medical, behavioral, and social needs.
• Onboard new patients, deliver care packages, and gather health readings.
