Job summary

Pair Team is expanding its field-based care management program in Alameda County. The Field Care Coordinator will bring care directly to patients at home and in the community, focusing on onboarding, support, and barrier removal to improve access to care.

The role requires bilingual English/Spanish abilities and a commitment to high-volume, in-person patient interactions five days a week. Gas reimbursements and a comprehensive benefits package are included.

Qualifications

• Bilingual – English/Spanish.
• 1+ years field-based (in-person) case management or care coordination.
• Physical location in or near Alameda County with familiarity of local resources.
• Reliable transportation for regular field visits.
• Valid driver's license and auto liability insurance.
• Cultural fluency and ability to build trust with diverse populations.
• Empathy and ability to work in a multidisciplinary care team.
• Excellent organizational skills and attention to detail.
• Comfort learning new systems (Google Suite, Slack, CRM, Excel).
• HIPAA-compliant workspace and documentation practices.

Responsibilities

• Manage a high-volume field-based caseload with approximately 30 in‑person visits per week.
• Travel throughout Alameda County to meet patients where they are.
• Support community events and provide care coordination for complex needs.
• Be the primary contact for members with complex medical, behavioral, and social needs.
• Meet members at homes, community sites, or partner organizations to onboard and deliver care packages.
• Conduct in‑person visits to assess needs, support plans, and adherence.
• Coordinate services and referrals across primary care, behavioral health, and social resources.
• Collaborate with Nurses and Behavioral Health Care Managers to ensure continuity of care.
• Identify barriers to care (transportation, access, health literacy) and address them.
