Job summary

Qualifications

• Bilingual – English/Spanish
• 1+ years of field based (in person) case management or care coordination
• Physical location in or near Alameda County
• Reliable transportation for regular field visits
• Valid driver's license and current auto liability insurance
• Cultural fluency and ability to build trust with diverse populations
• High degree of empathy and patience in adversity
• Strong organizational skills and attention to detail
• Experience with Google Suite, Slack, CRM databases, Excel, Word, email, and video conferencing

Responsibilities

• Manage a high-volume, field-based caseload with approximately 30 in-person visits per week (up to 120 per month)
• Travel throughout Alameda County to meet patients where they are
• Support community-based events and coordinate care across primary care, behavioral health, and social resources
• Serve as primary contact for members with complex medical, behavioral, and social needs
• Onboard new patients, deliver care packages, and collect health readings
• Document all interactions and care coordination activities
• Collaborate with nurses and care managers to ensure care continuity
• Identify and address barriers to care such as transportation, medication access, or health literacy
• Develop individualized care plans and ensure follow-up on interventions
