Job summary

The role emphasizes maintaining health records and coordinating services for child and family well-being. The ideal candidate will have a relevant degree and at least 1 year of client casework/case management experience, with bilingual capabilities and willingness to travel within the program

Qualifications

Candidate must be fluent in two or more languages, including at least one that serves the program community.
Experience in child recruitment, eligibility verification, parent orientation, and health records maintenance.
Valid California driver’s license, DMV record, and proof of insurance may be required for travel.

Responsibilities

Recruit and enroll children into the program.
Understand and adhere to program policies, confidentiality, privacy, and ethics.
Orient parents to comprehensive services including health, nutrition, education, disabilities, mental health, parent engagement, and ERSEA.
Link families with referral systems and community services; serve as a liaison for referrals.
Coordinate health, social, and educational services for children's learning and family well-being.
Transport families to meetings and health appointments when necessary.
