Job summary

North Los Angeles County Regional Center (NLACRC) seeks a Service Coordinator to assist individuals with developmental disabilities and their families. You will help develop and monitor service plans, coordinate community resources, and advocate for client needs.

The role requires working in community settings, flexibility for home visits or in-office meetings, and collaboration with a multidisciplinary team.

Qualifications

• Bachelor's degree in psychology, social work, sociology, or related human services field; at least two years of related experience.
• Master's degree in a related subject may substitute for experience.
• Knowledge of intellectual and developmental disabilities; strong written and verbal communication; proficiency with MS Word and MS Outlook.

Responsibilities

• Participate in developing program plans for individuals; monitor and evaluate plans and revise as necessary.
• Identify and coordinate services for individuals and their families.
• Documentation, completing forms and reports per regulations and NLACRC policies.
• Advocate for individuals served by community agencies.
• Schedule and attend interdisciplinary meetings with individuals served, families, service providers, advocates, etc.
