Job summary

North Los Angeles County Regional Center (NLACRC) is seeking a service coordinator to provide information, advocacy, and service arrangement for consumers and families within our community-based settings.

You will contribute to developing program plans, identify needed services, coordinate resources, and document case details while working with a diverse interdisciplinary team and maintaining compliance with NLACRC policies.

Qualifications

• Bachelor’s degree in psychology, social work, sociology, or related human services field; at least two years of related experience.
• A Master’s degree in a related subject may substitute for experience.
• Knowledge of intellectual and developmental disabilities; ability to work in interdisciplinary teams.

Responsibilities

• Provide information, advocacy, and service arrangement for consumers and families.
• Participate in developing, monitoring, and revising program plans for individuals.
• Identify and coordinate services for individuals and families.
• Document forms and reports per regulations and NLACRC policies.
• Serve as an advocate for individuals served by community agencies.
• Attend interdisciplinary meetings with individuals, families, providers, and advocates.
• Rotate officer-of-the-day duties and provide emergency on-call services.
