Job summary

NLACRC is hiring a Service Coordinator to provide information, advocacy, and service arrangement for consumers and families in the San Fernando Valley area. The role involves developing and monitoring individual plans, coordinating services, documenting activities, and serving as an advocate across multiple agencies.

Candidates should have a bachelor’s degree in a human services field and at least two years of related experience; a master’s degree may substitute for experience.

Qualifications

• Bachelor’s degree in psychology, social work, sociology, or related human services field; two years of related experience required.
• Master’s degree may substitute for experience.
• Knowledge of family systems and person-centered planning; strong communication skills; ability to work independently and in a team.

Responsibilities

• Provide information, advocacy, and service arrangement for consumers and families.
• Participate in development and evaluation of individual program plans.
• Identify and coordinate services for individuals and families.
• Complete documentation and reports per NLACRC policies.
• Advocate for individuals served by community agencies.
• Attend interdisciplinary meetings with individuals served, families, providers, and advocates.
• Rotate duty as officer-of-the-day; provide on-call support when required.
