Qualifications

Bachelor’s degree in Social Work, Psychology, Sociology, or related field.
2+ years in community resource information & referral, case management, or peer support.
Bilingual in Spanish required.
Experience with Salesforce or CRM systems preferred.

Responsibilities

Provide referrals, information, and intervention services to address client needs.
Maintain non-judgmental customer service and connect people to services.
Conduct follow-ups and track outcomes of referrals.
Enter and manage data in Salesforce and other systems.
Build relationships with providers and community partners.
Assist with program quality improvement and reporting.
