Job summary

CLUES in St. Paul, MN is seeking a Community Resource Navigator for the Creando Puentes program to outreach to the Latino community and connect individuals with needed mental health services in English and Spanish.

The role provides education, case management, home visits, transportation support, and collaboration with Ramsey County Human Services; the position is full-time with a comprehensive benefits package including medical, dental, vision, life insurance and paid holidays.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, public health or related field.
• Bilingual in Spanish & English with cultural sensitivity.
• Strong communications skills and ability to build relationships.
• Must have reliable transportation and valid driver's license.

Responsibilities

• Conduct outreach and engagement to Latino community to connect individuals with services.
• Provide case management and assist access to services, including transportation.
• Educate clients about mental illness and available services in English and Spanish.
• Maintain up-to-date client records and confidentiality per HIPAA and policies.
• Collaborate with multi-disciplinary team to support clients and refine program.
• Support program reporting and data collection for outcomes.
