Job summary

Primary Care Coalition (PCC) in Silver Spring, MD, seeks a Bilingual Community Resource Coordinator (L1) to join the Montgomery Cares Behavioral Health Program. This full-time role supports patients by connecting them with health and social services, coordinating referrals, and assisting care managers.

Fluent in English and Spanish, you will provide patient navigation, outreach, and resource research, while documenting encounters in the EHR and collaborating with partner organizations and PCC

Qualifications

• High School graduate or GED required.
• At least one year of experience in health, human services, customer service, patient support, or related setting preferred.
• Experience working with diverse, low-income, or immigrant communities preferred.
• Knowledge of behavioral health or medical issues preferred.
• Community Health Worker licensure preferred.

Responsibilities

• Provide patient navigation and support services to patients as part of treatment plans.
• Refer patients to community or social service agencies.
• Complete home and community-based visits to help patients access benefits and services.
• Assist with applications (e.g., SNAP, Montgomery Cares).
• Document all patient encounters accurately in the electronic health record and case management system.
• Collaborate with healthcare and social service providers and PCC teams.
• Assist care managers with behavioral health screenings for anxiety and depression.
• Share information about programs and participate in networking with stakeholders.
• Attend trainings to improve skills and learn about new resources.
