Job summary

Los Angeles Christian Health Center is seeking an Outreach Care Coordinator to support its mission by leading care management for the Medi-Cal ECM program, coordinating biopsychosocial needs and expanding patient enrollment.

You will manage an ECM caseload, apply motivational interviewing, facilitate referrals, assist with scheduling, and engage with community partners to promote services. Strong English skills with Spanish preferred; CPR may be required.

Qualifications

• Two years of case management experience required.
• Bachelor’s degree in social work or related field preferred.
• Bi-lingual Spanish preferred; CPR certification may be required.

Responsibilities

• Lead care management for the Medi-Cal ECM program.
• Coordinate biopsychosocial care for enrolled patients.
• Engage in community outreach and patient enrollment efforts.
• Represent the organization at community and faith-based events.
• Assist with referrals, scheduling, and care coordination activities.
• Utilize evidence-based outreach strategies to link clients to care.
• Provide culturally appropriate health education and goal setting.
