Job summary

Emanate Health seeks a Community Health Worker to engage patients, navigate social services, and support clinical staff across mobile clinic sites in our community. This frontline role requires travel to streets, shelters, schools, and centers, with sensitivity to diverse populations.

The CHW will build trusting relationships, provide culturally responsive support, and help patients access needed resources to improve health and stability.

Qualifications

• High school diploma or equivalent required; health/social services education preferred.
• Experience with underserved populations; strong communication and trust-building.
• Knowledge of local resources and social service systems preferred.

Responsibilities

• Engage patients and families in outreach and navigation.
• Support clinical staff in delivering care across sites.
• Travel to multiple sites including outdoor, mobile environments.
• Document encounters and coordinate with community resources.
