Mission Action, a community organization in San Francisco, seeks a Case Manager (Bilingual Spanish) for the DSP program. The role requires completing intake assessments, data entry in RTZ OneSystem, and coordinating care with clients onsite, staff, and city partners.

The position is full-time, non-exempt, paying $28 per hour with a broad benefits package, including medical, dental, vision, life insurance, FSA, commuter benefits, generous paid leave, and a 401(k) with employer matching.

Qualificações

• Spanish fluency strongly preferred, including ability to provide written translation of documents.
• Proficient in Microsoft Word and Excel.
• Well organized, efficient, highly motivated, and able to handle multiple tasks at once, work well under pressure, and take initiative.
• Experience working with low-income, immigrant, and monolingual Spanish-speaking communities.
• Approach work from a trauma informed lens.

Responsabilidades

• Complete client intake assessments and necessary documentation for all new clients of the site
• Enter data daily into the RTZ system, OneSystem as per contract requirements
• Report on Slack! And review site care coordination with site manager informed
• Conduct client wellness checks to ensure the safety and well-being of all clients onsite
• Provide care coordination, crisis intervention, and conflict resolution with clients
• Work collaboratively with Mission Action staff and city staff to ensure effective coordination and operation of the site
• Other duties as assigned
