Job summary

Holy Family Institute in Moon Township is seeking a Bilingual Case Manager (Spanish/English) for the Long Term Foster Care program. The role coordinates intake assessments, develops individualized care plans, and ensures documentation and compliance with ORR and DPW guidelines.

You will work with a multidisciplinary team to support minors, ensure safety, and coordinate family reunifications, transfers, and discharge planning.

Qualifications

• Bachelor's degree in behavioral sciences, human services or related fields.
• Must be fluent in both English and Spanish (read, write, speak).
• Excellent written and verbal communication skills.
• Act 33/34 and FBI Clearances required.
• Must be at least 21 years of age.

Responsibilities

• Coordinate case management services for LTFC program.
• Develop intake assessment and individualized care plans.
• Ensure proper documentation in each child’s case file.
• Coordinate release and referrals as needed.
• Attend weekly meetings with ORR representatives.
• Travel when needed for program activities and hold on-call rotations.
