Job summary

Clear Destination Inc. is seeking a Bilingual Family Support Specialist to manage a caseload in the Foster Care program, providing direct services and care coordination. The role involves advocacy between families, DCF, schools, probation, and other supports, and may include group facilitation.

The position requires a BA/BS in a related field and bilingual capabilities. A valid driver’s license and experience with mental health services are essential.

Qualifications

• BA/BS degree in Psychology, Social Work, Marriage and Family Therapy, Counseling or related field is required.
• Previous experience with mental health and/or co‑occurring substance use disorders.
• Effective time management and strong verbal and written communication skills are required.
• Valid Driver’s License is required; ability to operate a vehicle in the community.

Responsibilities

• Develops case management plans and coordinates care for assigned clients/families.
• Provide front line crisis support by phone, in-home, or in the community.
• Coordinate treatment phases with internal and external service providers and family supports.
• Maintain records of counseling and coordination of care, and ensure documentation complies with Medicaid and licensing requirements.
• Communicate effectively with clients, families, and providers; participate in supervision and team meetings.
