Job summary

Catholic Charities is seeking a Case Manager for St. Michael's Home for Children in Houston. You will coordinate all case management services, support family reunification, and ensure timely assessments and treatment plans for clients in care.

You lead the treatment team, maintain documentation, and communicate with families, sponsors, and external agencies. A Bachelor's degree and bilingual English/Spanish are required.

Qualifications

• Bachelor's degree in behavioral sciences, human services or social services.
• 1 year of experience in child welfare/residential and/or adolescents in a social service setting.
• Bilingual in English and Spanish.
• Valid Texas Driver’s License.

Responsibilities

• Maintain a caseload per agency policies and licensing and contract standards.
• Conduct assessments and gather information for placement and family reunification.
• Lead the weekly treatment team and provide case management to ensure client needs are met.
• Complete documentation of client progress within required timeframes and maintain records.
• Communicate with families, sponsors, and outside entities; coordinate referrals as needed.
• Maintain reliable case management databases and ensure timely data entry.
