Job summary

LifeMoves in Palo Alto is seeking a Case Manager for the Opportunity Services Center to support unsheltered adults with housing, benefits enrollment, and connections to healthcare and community resources. You will develop individualized plans, assist with housing searches, and advocate for clients with partners and landlords in a trauma-informed, client-centered approach.

Qualifications

• Associate's degree preferred; or HS diploma/GED with 2-5 years related experience.
• Fluency in Spanish strongly preferred.
• Trauma-informed, empathetic approach required.

Responsibilities

• Develop and maintain individualized case plans focused on housing, employment, benefits, and wellbeing.
• Assist housing searches, benefits enrollment, budgeting and daily living skills.
• Connect clients to healthcare providers and community resources; coordinate transportation as needed.
• Document and track outcomes in HMIS; advocate with partners and landlords.
