Job summary

Sunnyside Community Services, Inc. is seeking a Bilingual Case Manager to assist impaired older adults in maintaining independent life in the community.

The role links clients to home services, advocates for benefits, and coordinates care plans with families while conducting home visits and staying in touch by phone. The position operates on a hybrid model with three in-office days and two remote days, offering comprehensive benefits and opportunities to collaborate with community and government

Qualifications

• MSW/BSW in human services with two years of supervised experience.
• Bilingual proficiency in Spanish, Bengali, Greek, Cantonese, or Mandarin required or preferred.
• Driver preferred.

Responsibilities

• Screen requests for services, determine eligibility, provide information and referrals.
• Conduct home assessments to determine needs and plan of care for home services.
• Maintain contact with clients, caregivers, and families; monitor status and intervene as needed.
• Advocate for clients regarding benefits and entitlements.
• Refer clients to emergency services when necessary.
• Develop and maintain contacts with community and government agencies.
