Job summary

Greater Lynn Senior Services, Inc. is seeking a dedicated Case Manager to help older adults and people with disabilities access services and live independently. The role involves in-home assessments, care plan development, and coordinating community-based supports.

Full-time, in-office for the first 90 days with later in-office community visits; bilingual abilities valued; strong communication and documentation skills essential; valid driver’s license required.

Qualifications

• Bachelor's degree in human services, social work, or related field preferred. Minimum of one year experience in the human services field in lieu of Bachelor's Degree with waiver possible.
• Bilingual in Haitian Creole, Spanish, or Russian preferred
• Proficient computer skills
• Expert time management with the ability to adjust to changing priorities
• Strong desire to help others and advocate for those needing services and assistance
• Excellent communication skills with the ability to establish positive and professional relationships with team members, providers, consumers, and their support system
• Strong documentation skills with the ability to complete documentation timely, accurate, and comprehensive
• Ability to handle confidential and personal information per HIPAA standards
• Demonstrates strong work ethic, dependability, and meeting deadlines
• Valid Driver's License and reliable transportation for home visits throughout the GLSS service area

Responsibilities

• Conduct in-home assessments to determine needs and create person-centered care plans, coordinating and monitoring community-based services
• Identify, coordinate, authorize, secure and monitor services as needs change
• Modify and adjust care plans based on consumer needs or situation changes
