Sunnyside Community Services in New York is seeking a Bilingual Case Manager to help impaired older adults safely maintain independent life by linking them to community resources and home services, and advocating to obtain benefits.
You will work with clients and families to formulate appropriate care plans, conduct home visits, and maintain contact by phone. The position is hybrid: three days in office, two remote.

The person in this roleworks with clients and their families to formulateappropriate care plans, maintaining contact by phone and conducting home visits.
This position currently operates on a hybrid work model with three days in the office and two days remote.
Bending, sitting, stooping, standing, and walking up and down stairs.
The individual in this position will work in a typical office setting, with field visits that may potentially expose them to client illnesses.
